Application Form 2022-23

1. For OLD students, please return this form, together with PHOTO to the Parish office BEFORE NOON 30th APRIL, 2022.
2. For NEW students, please attach a copy of your child’s Baptism Certificate, Address Proof together with PHOTO and return by hand BEFORE NOON 28th MAY, 2022 to the Parish Office marked: ATTN: FR. GREGOIRE VIGNOLA.
3. Kindly attach your contribution of HK$500 PER CHILD; cheque made payable to: EVA LIU. This contribution will help us cover the cost of teaching and study materials, library books, and venue hire, etc.
4. Successful applicants will be notified by email.
